What a Medical Virtual Assistant Does, What to Delegate First and How to Keep It HIPAA-Compliant

What they actually do

Front desk and scheduling

  • Answer and return calls, schedule and reschedule, manage the waitlist and fill cancellations.
  • Appointment reminders and confirmations by phone, text and portal.
  • New-patient intake: demographics, insurance capture, forms sent and chased before the visit.

Insurance and authorizations

  • Eligibility and benefits verification before every visit, with copay and deductible noted on the appointment.
  • Prior authorization requests, follow-up and tracking.
  • Referral management, inbound and outbound.

Clinical support (non-clinical tasks)

  • EHR updates: scanning and indexing documents, updating medication and problem lists from outside records under provider direction, routing results.
  • Prescription refill request triage by protocol.
  • Pre-visit chart preparation and post-visit follow-up calls.
  • Virtual scribing during visits, where the practice chooses it.

Back office

  • Patient statements and balance calls.
  • Credentialing document gathering and CAQH maintenance.
  • Inbox and fax management, portal messages triaged to the right person.

What to delegate first

Pick the task that is both high-volume and well-defined. For most practices that is appointment reminders and confirmations, then eligibility verification, then intake. Write the procedure down (or let the assistant write it from watching your staff for a week), set the hand-off rules, and measure one thing: no-shows, or time to first available appointment, or how many calls the in-office staff still take. Add the next task once the first is steady.

Keeping it HIPAA-compliant

  • Business associate agreement with the staffing company, and HIPAA training for every assistant, documented.
  • Access by role. The assistant gets a named user account in your EHR and practice management system with the permissions the role needs, nothing more, and it is removed the day the engagement ends.
  • No data on local devices. Work happens inside your systems through secure remote access; no downloads, no local storage, no personal email.
  • Secure workspace. A private room, a locked screen, headphones for calls, and a company-managed device.
  • Audit trails. Your systems log what each user did. Review access periodically, as you would for in-office staff.
  • Minimum necessary. Scripts and procedures that limit what is discussed and shared to what the task needs.

Onboarding that works

  1. Define the role in a one-page scope: tasks, hours, systems, who they report to.
  2. Provision accounts and remote access before day one.
  3. Shadow week: the assistant watches the in-office workflow and writes the procedure.
  4. Supervised week: the assistant does the work with a named in-office contact reviewing.
  5. Steady state: a weekly fifteen-minute check-in and a monthly look at the numbers.

Measuring the result

Choose two or three of: no-show rate, abandoned calls, days to third next available appointment, percentage of visits with eligibility verified before arrival, prior authorizations completed before the service date, time the in-office team spends on the phone. Compare the month before the assistant started with the third month after. If nothing moved, the task definition is wrong, not the assistant.
